A Collaborative Resource for Your Recently Discharged Patients

The Post-Acute Care Clinic (PACC) at Regional Medical Center is a short-term, transitional clinic designed to support patients during the critical days immediately following discharge. The clinic provides timely follow-up care for patients who are unable to secure a quick appointment with their primary care provider or who do not yet have one.

Our role is to bridge the gap between hospital care and a patient’s return to their medical home, ensuring patients receive the early support they need to stay on track with recovery before transitioning back to you for ongoing care.

How the PACC Supports Your Patients

Patients seen in the PACC receive a single Transition-of-Care visit focused on early recovery and care coordination. During this visit, our clinical team will:

  • Review discharge instructions
  • Reconcile medications
  • Reinforce patient education
  • Address early recovery questions or concerns
  • Coordinate next steps back to the patient’s established PCP or assist with establishing one

This early intervention helps support safer recoveries, improve continuity of care, and reduce avoidable readmissions.

Our Commitment to You

The PACC is an adjunct service, not a replacement for primary care. Patients are only seen when immediate post-discharge follow-up is unavailable. Once the Transition-of-Care visit is complete, patients are returned to their established provider or guided into a new medical home based on their needs and location.

Our goal is to support you by ensuring your patients remain stable and informed during the most vulnerable phase of recovery.
