Create an account using Chartfast to electronically request to receive your entire hospital medical record. If you encounter issues with creating an account, security questions, passwords, or if you get locked out of your account, etc., please call our Release of Information Center at 256-235-5763.

Generally, you can expect to receive your records within 2-5 business days.
In order to pick up another person’s records you must have written permission from the patient and bring a copy of the patient’s photo ID and your own ID. If you have guardianship or power of attorney of the patient, you must also present that document.
A copy of the letter of Testamentary naming the Executor of the Estate or Letters of Administration naming a personal representative of the deceased is required. You will also need to bring a copy of the death certificate.

We may charge you the following fees when you request your records for your own personal use: $6.50 flat fee for “Patient Directive Requests”.
Yes. With your permission, the physician office can contact us for copies of your records.
