Electronic Request for Records

Create an account using HealthMark Group to electronically request to receive your entire hospital medical record. If you encounter issues with creating an account, security questions, passwords, or if you get locked out of your account, etc., please call our Release of Information Center at 256-235-5763.

Medical Record FAQs

Generally, you can expect to receive your records within 2-5 business days.

In order to pick up another person’s records you must have written permission from the patient and bring a copy of the patient’s photo ID and your own ID.  If you have guardianship or power of attorney of the patient, you must also present that document.

A copy of the letter of Testamentary naming the Executor of the Estate or Letters of Administration naming a personal representative of the deceased is required. You will also need to bring a copy of the death certificate.

You can visit the RMC Radiology Department on the 2nd floor or for films at

You can visit the Radiology Department on the 2nd floor of the main hospital.  Please make sure you bring the proper ID with you.

There is no fee associated with request for records.

Yes.  With your permission, the physician office can contact us for copies of your records.

You may request a copy of your bill by going to the Cashier Window on the third floor.

You may request a copy of your bill by going to the Business Office on the first floor.
